Box Tops for Education

Roy Elementary participates in mandated DIBELS and RISE testing. If you would like your child to be opted out from testing,
for the paperwork that will need to be filled out and returned to the principal.

To fill out the free and reduced meal form, go to and sign in, click on "Tools", then click "Meal Application". If you have questions, please contact the office.

Breakfast and lunches will be free to all this year, but we still need everyone to fill out the free and reduced meal form.
This is how we get federal funding to have four more teachers and additional funding for our school.

My Role as the School Nurse:

My goal is to keep our students healthy, safe, and ready to learn. I work closely with our school counselor, mental health specialist, the principal, and teachers to identify any health concerns, and provide resources to assist parents to access health care when needed.

Health Care Plans and Medication:

The following link will take you to the WSD Medication Policy:

If your child will need medication at school, including prescription and over the counter medication, please have your health care provider fill out the following form.

If your child has any of the following health care issues, please have your provider fill out the appropriate health care plan for your child:

Other applicable forms can be found here:

School Screenings:

State guidelines require that we do a vision screening on students in grades PK, K, 1, 3, and 5 at the elementary level. If your child does not pass the vision screening at school, I will notify you by mail. If you cannot afford to get your child’s vision screened by an eye care professional, please notify me. There are resources in the community to get a free vision screening and glasses if you qualify financially.

Local Health Resources:

I have attached a link for the Weber-Morgan Area General Resource List to help with low cost health clinics, and other assistance programs.
